HOW THIS SERVICE WORKS
True North Wound Care at Home provides nursing services within scope of practice, focused on in-home wound and ostomy assessment, education, and clear care planning.
PRIMARY SERVICES:
Wound Care
Services may include nursing assessment and support for:
- chronic or non-healing wounds
- venous leg ulcers
- arterial and mixed-etiology ulcers
- diabetic foot wounds
- pressure injuries
- surgical or post-procedural wounds
Vascular Assessment & Compression Therapy
Many lower-limb wounds are closely linked to chronic edema, venous insufficiency, lymphedema, or impaired circulation. In these situations, early assessment of circulation and edema can help reduce the risk of wound development in at-risk individuals and plays an important role in guiding safe, effective care.
When appropriate, services may include:
- lower-limb vascular assessment
- Ankle-Brachial Pressure Index (ABPI) testing to assess arterial perfusion
- clinical determination of whether compression therapy is appropriate
- education regarding different compression options, including wraps or garments
- integration of compression planning into the overall wound and edema management plan
- guidance on safe compression levels (e.g., light, moderate, or therapeutic)
Compression recommendations are always guided by assessment findings, clinical history, and individual tolerance, and are used to support safe, evidence-based care planning and communication with other healthcare providers.
Each visit focuses on understanding the full clinical picture, including relevant medical history, medications, mobility, lifestyle factors, and how the wound has been managed to date.
When appropriate, evidence-based nursing assessment frameworks and validated tools may be used to support care planning and communication with other healthcare providers.
ADDITIONAL SUPPORT: Ostomy Care
Ostomy care is offered as a secondary service.
Support may include:
- ostomy assessment and troubleshooting
- peristomal skin assessment
- pouching challenges and leakage issues
- education related to ostomy care and management
Each visit focuses on understanding the full clinical picture, including relevant medical history, medications, mobility, lifestyle factors, and how the wound has been managed to date.
The goal of ostomy visits is to improve comfort, function, and confidence through practical nursing assessment and education.
SELECT SUPPORT: Continence-Related Skin Care
Select continence-related support is provided within nursing scope of practice, with a focus on skin protection and education.
This service does not replace specialized continence clinics or medical management.
SERVICE FEES
Effective wound care often occurs over more than one visit.
The initial visit focuses on assessment and education.
Follow-up visits, when recommended, focus on reinforcement and adjustment based on wound response over time.
Initial In-Home Assessment

Most clients begin with an initial in-home nursing assessment, scheduled for up to one hour.
This visit focuses on:
- review of relevant medical history and medications
- assessment and documentation of the wound or concern
- discussion of current wound management
- education for clients and caregivers
- identification of factors that may affect healing
- discussion of reasonable care options and next steps
Education is a core part of every visit. Understanding why something is being done supports informed decision-making and confidence in care.

Follow-Up Visits
Fees: discussed based on individual need
(Reinforcement & Adjustment)
Most clients begin with an initial in-home nursing assessment, scheduled for up to one hour.
Follow-up visits may be recommended to:
- reassess wound progress
- confirm response to care strategies
- reinforce education and techniques
- adjust nursing recommendations as appropriate

Same-Day Availability (Non-Emergency)
In some situations, same-day visits may be available for time-sensitive wound concerns causing uncertainty or distress.

Services are provided throughout: Midland, Penetanguishene, Orillia, Barrie and surrounding areas of Simcoe County.
Travel beyond the core service area may be available and discussed in advance.
